I did a Google search and this appears to be it or at least close. OP correct me if I'm wrong.
I've made this dish a few times, that's the exact recipe I use. It's always a big winner with everyone.

Does anyone know the history behind this dish? The name implies it's Italian, but the amount of sauce and particularly heavy cream is kinda unusual to me for Italian cuisine.
Def not Italian. Never heard of it. The traditional Tuscan chicken recipe is "pollo alla cacciatora" and has nothing to do with this
